If intraarterial infusion is carried out through an infusion catheter, depending on the tolerance and severity of the disease, a dose of 0.1 - 0.6 ng/kg/min is recommended with 12 hours of administration (corresponds to 0.25 - 1.5 ampoules of Vasaprostan).

 

 

Intravenous infusion

 

Dissolve the contents of 2 ampoules of Vasaprostan lyophilizate (corresponds to 40 µg alprostadil) in 50-250 ml of physiological solution and inject the resulting solution intravenously for 2 hours 2 times a day or 3 ampoules (60 µg alprostadil) for 3 hours 1 time a day.

 

In patients with impaired renal function (renal failure at creatinine level more than 1.5 mg/dL) intravenous injection starts at 20 µg for 2 hours. If necessary, after 2-3 days a single dose is increased to 40-60 micrograms.

 

For patients with renal and heart failure, the maximum volume of injected fluid is 50-100 ml/day. Course of treatment - 4 weeks.

 

The duration of treatment - 14 days on average, in case of a positive effect the treatment can be continued for another 7-14 days. If there is no positive effect within 2 weeks from the start of treatment, further use of the drug should be stopped.

The solution must be prepared immediately before infusion. Lyophilizate is dissolved immediately after adding the physiological solution.

At the beginning, the solution may turn out to be milky muddy. This effect is created by air bubbles and is not important.

After a short time, the solution becomes transparent.

The solution prepared more than 12 hours ago should not be used.

Symptoms:

Decreased BP, increased NHS. Vasovagal reactions are possible with pale skin, increased sweating, nausea and vomiting, which may be accompanied by myocardial ischemia and symptoms of heart failure, and pain, swelling and redness of the tissue at the infusion site.

 

Treatment:

The dose should be reduced or the infusion stopped. If there is a marked decrease in BP, the patient should lift his legs lying down. If symptoms persist, it is necessary to use sympathomimetics.

 

• Hypersensitivity to alprostadil or other components of the drug;

 

• Chronic heart failure of functional class III-IV according to NYHA;

Digestive system: a sense of discomfort in the epigastric region, dyspeptic phenomena - diarrhea, nausea, vomiting.

 

Musculoskeletal system: hyperostosis of long tubular bones (in treatment for more than 4 weeks).

Very rarely (up to 1% of cases): shock, acute heart failure, hyperbilirubinemia, bleeding, drowsiness, bradypnoea, reduced respiratory function, tachypnoea, anuria, renal impairment, hypoglycemia, ventricular fibrillation, AV blockade II degree, supraventricular arrhythmia, neck muscle tension, increased irritability, hypothermia, hypercapnia, hyperemia of the skin, hematuria, peritoneal symptoms, tachyfilaxia, hyperkalemia, thrombocytopenia, anemia.

 

Allergic reactions: skin rash, itching.

 

Side effects associated with the application of the drug or the procedure of catheterization itself, disappear after a dose reduction or cessation of infusion.
