Vivitrol is prescribed for:

In order to ensure accurate dosing, the instructions for preparation and administration of the preparation should be clearly followed. Aseptic rules must be followed.
1. Suspension preparation kit composition
- 1 vial with solvent;
- 1 vial with powder;
- 1 disposable syringe;
- 2 needles for injection with protective cap;
- 1 short needle for suspension preparation.
2. Procedure for the preparation of the suspension
- Before use, take the product pack out of the fridge and let it warm up to room temperature (approx. 45 min);
- for easy mixing, knock the tip of the powder bottle on a hard surface to open the powder;
- remove the aluminium caps from both vials (do not use the product if the vial caps are damaged or missing);
- wipe the necks of the bottles with an alcoholic cloth;
- put a short suspension needle on the syringe and remove 3.4 ml of solvent from the solvent bottle (some amount of solvent will remain in the bottle);
- inject 3.4 ml of solvent into the powder bottle;
- mix solvent and powder by shaking the vial intensively for approx. 1 min. Before continuing, make sure that the suspension is homogeneous. A properly mixed slurry must be milky white, free of lumps and flow down the walls of the vial.
3. Suspension filling order
- Immediately after preparation, remove 4.2 ml of suspension in a syringe using a suspension needle;
- take that needle off and put an injection needle on the syringe;
- before the injection, knock on the syringe to release air bubbles, then gently press the piston until there is 4 ml of suspension left in the syringe. The suspension is ready for immediate injection.
Procedure of injection of suspension
- The needle of the syringe is injected deep into the buttock muscle; after that, the suction movement of the piston checks if the needle has got into the vessel (if it has got into the syringe, blood is thrown into it). If there is blood, the procedure should be repeated by replacing the needle with a spare one;
- the drug is injected in a gentle movement deep into the buttock muscle. The suspension is injected alternately in different buttocks.
Vivitrol cannot be injected intravenously or subcutaneously.
Disposal of used and unused packaging components
- after injection, close the needle with the protective cap, pressing it with one hand against a solid surface and holding it in the direction of the needle (using the protective cap prevents splashing of liquid that may remain on the needle after injection);
- dispose of used and unused packaging components.
The overdose data is very limited. A single dose of 784 mg was administered to 5 healthy volunteers. They had no serious side effects.
Symptoms:
the most common side effects were reactions at the injection site, nausea, abdominal pain, drowsiness and dizziness. No significant increase in the activity of hepatic enzymes was observed.

Cautiously
Use in cases of liver insufficiency:
Patients with mild to moderate liver dysfunction (Child-Pugh grades A and B) do not require dose correction. Patients with severe liver dysfunction have not studied the pharmacokinetics of Vivitrol. Such patients, like all other Vivitrol injections, should be administered with caution due to the risk associated with the Vivitrol injection (e.g. thrombocytopenia and coagulation disorders).
Application in case of renal failure:
Patients with mild renal failure (Cl creatinine 50-80 ml/min) do not require dose correction. Vivitrol pharmacokinetics has not been studied in patients with moderate and severe renal insufficiency. Since naltrexone and its primary metabolite are mainly excreted in the urine, it is recommended to prescribe Vivitrol with caution to patients with moderate or severe renal failure.
